Jenna Dewan Wants Her Maiden Name Back

After her divorce from Channing Tatum in November of 2019, Jenna Dewan is looking to change her legal name back to her maiden name. According to documents obtained by TMZ, Jenna asked the court for permission to change her last name, as her divorce settlement was silent on whether she could revert back to her maiden name.

In Ontario, you can change your last name to reflect your married name or maiden name without having to go to court. Pursuant to section 3(2) of the Change of Name Act, a person can elect to resume the last name they had immediately before the marriage at any time after their marriage is dissolved by divorce, annulment or death. The process is fairly straightforward and requires the filing of an Election to Resume Former Surname form, which only costs $25. Once the paperwork is completed, you get a certificate of name change and a new birth certificate, if you were born in Ontario, and you will be able to use them to update your other government-issued IDs.

On the other hand, if you wish to assume your spouse’s last name upon marriage, there is no cost involved and you would simply visit a ServiceOntario Centre with your marriage certificate, current photo health card and driver’s license or Ontario Photo Card.
