Form 15C - Consent Motion to Change

Use this form when both parties have agreed to change or terminate a final order or support agreement for something other than just child support. You can complete this form at any time during your case, if you and the other party reach an agreement. If you have agreed to change a previous order or agreement for child support only, you should complete Form 15D: Consent Motion to Change Child Support instead. If you have agreed to change a previous agreement (and not a court order), you should use this form to change the terms dealing with support only. If you wish to make other changes (such as custody or access), you will have to complete Form 8: Application. If you want to make changes to support terms in a previous agreement (and not a court order), you must file the agreement with the court before you file this form. Note that you only need to fill out one form. Both parties will sign the same form.
